Admission Criteria/Referral Process:

If you have acquired a serious head injury and would like to have support, we now provide Personal Support Workers. Here is a list of the process for the services we provide for your head injury claim:

• Personal interview with Sue Smith
• Pediatric through geriatric
• Completion of a client-based interview package
• Documentation of individualized medical needs
• Payment is accepted from WSIB, private insurance, MVA claims, post settlement dollars, or personal cheques.

If a Treatment Plan is required, NEW BEGINNINGS will provide information including Section 11 (identifying the Health Providers), and Section 12 (Proposed Goods and Services).and soon HST
